Thanks Sir, I do not have much idea. I would proceed this way. I am very much junior to you and I may be entirely wrong. I would first consider from what the patient wants.

Mr.K. says he wants to die.

As far as India is concerned, no one has the right to die. As you said earlier he may be depressed. The next question is if he is competent or not.

    1)If he is competent?

Can Mr.K. refuse feeding jejunostomy? Absolutely Yes. Pts. have the absolute right to refuse any treatment. Some religious groups have the right to refuse blood transfusion, even if it is the only option available to save a pt. If he refuses, one of the possible options is TPN for short term and we can wait for him to give consent

    2) If he is not competent ?

It is in the pt's best interest to do jejunostomy and a consent from any one of the close
relatives/care givers shd be sufficient. Since the daughter is ready to give the consent, I think we can proceed with the surgery. I am not sure if th...    less

And in this particular case if we think theoritically, then in meta-analysis it has been shown that TPN has no effect on mortality in critically ill patients, so we can`t predict in this patient about his survival time. This is my opinion, would like to know yours.  less

Scenarios are better compared to theories that we recall and jot down here.they extract a good deal of reasoning and of course everybody of us enjoys answering one.So post questions and answers, experienced(heard) and synthesized, possible and impossible, torment your head and others' too.Doesn't matter which subject it concerns, neonatology to thanatology, everything will do.

Most doctors are book worms. They read medical and non-medical books extensively. (I read one book a week).

I have started this group so everyone can write reviews about the books they read and help fellow doctors in choosing what to read.

Start a thread (with the book name and author as title) and write your review about the book(medical, novels, non-fiction etc)

Others can add their reviews below or add thier comments about the review in the same thread.
